We accept referrals from Clinical Commissioning Groups (CCGs), local authority learning disabilities teams, hospitals, social services and direct referrals.
To refer someone for our services, please email admin@luminalivingcare.co.uk When we receive your initial referral, we will send you our referral form to complete within 24hours. Once this is received, we will arrange for our care Manager to carry out a comprehensive needs/risk assessment.
The assessment is strength based and we determine if our homes are suitable for you to achieve the most positive outcomes. We assess your personality, skills, abilities, what’s important to you, and what your goals are. We use this information to determine the level of support you need from us, including any specific housing requirements. We work with you to adapt your preferred service to support your independence and autonomy.
Family and Care Manager visits are arranged so they can meet our care team and see our service for themselves. When the placement is agreed by you and the care manager, the care manager will then apply for the funding, the start date is agreed and a transition plan is created, to ensure that the transition is as smooth as possible.
We have quick turnaround time on setting up services and in some cases may be able to provide you with a service within three weeks of the initial referral.
